Effective risk management should be an ongoing process rather than a one-time activity. Your process should include identifying areas of moderate to high risk early in your development process, and then taking steps to mitigate those risks/hazards as soon as possible.

Inevitably, some defects may make it into the product and cause some form of deviation from the expected results. In these instances you must analyze the product and determine why the deviation occurred. To close the loop on risk management, you must also document the cause of the deviation and use that information as an input to future product releases and versions—ensuring your continuous improvement goals are met.
