Bug or Feature? Foundational Dysfunctions

I'm obviously "a fan" of open source foundations. That doesn't mean they are simple and everything is always right. Actually, I wanted to ask whether anyone has noticed "organizational dysfunctions" with open source foundations. For example, only the Apache Software Foundation is really rigorously enforcing diversity in projects. Other foundations are less demanding, and hence existing developers can shut out potential contributors if they don't like them, for example, if there are commercial motives involved. Anyone got some more examples?
