It is becoming difficult to keep service provider?s environment as reliable and effective for the various network services, while user requirements are complicated and diversified. In result, there much focus and concern on the technology of Policy-Based Network Management (PBNM). In the case of IP services, in particular, PBNM has been regarded the one of ways to solve complexities for managing various Network Element, QoS on different services, security and etc.

We classify policy as three levels based on QPIM information flow such as service level, network level and device level policy. In relation to the definition of policy information, the research about device and network level policies has been done. However, it has the limitation to directly apply these information models to the service management system for service providers. In this paper, we will define service information model for guaranteeing the SLA of various services using the device level and network level policy information. We will also design the service management system based on this information model
